To schedule your first appointment, please call our office at (541)747-4300. During the phone conversation with our office staff, you will be asked for an email address. Please provide a valid address, as we will use it to send you your patient registration information, including a temporary username and password. All new patient paperwork is filled out directly on the portal. This will allow you to check-in for your appointment with all of the necessary paperwork already being completed. Please arrive to your first appointment 10 minutes early and bring a photo ID and current insurance card.

 

If you do not have an email address, and would prefer a mailed copy of your new patient brochure and registration paperwork, please provide your mailing address during the phone conversation with our office staff. Please bring the completed paperwork with you to your first appointment, along with a photo ID and your current insurance card. Please arrive 10 minutes early to your first appointment so we may check you in. You may also complete and download the new patient forms by clicking on the "New Patient Packet" link.
