BEHAVIORAL HEALTH


Your mental health is important to us, and we provide integrated behavioral health care within your medical home.

Integrated behavioral health care involves behavioral health providers working closely with your primary care provider to address aspects of overall health and wellness. We understand that many medical conditions often have a significant impact on our mental health and for that reason our team is located directly within our clinics to increase accessibility and improve overall health outcomes. 

Our Team:
Behavioral health providers: Our providers are composed of licensed psychologists, social workers, and clinical social work associates who provide brief interventions to address aspects of health-related behaviors impacted by depression, anxiety, and trauma. They can also assist with specific health behaviors related to diet, physical activity, smoking cessation, chronic pain, and sleep.

Behavioral health care managers: Care Managers work closely with our patients and our patient’s families to coordinate community resources, referrals, and scheduling to ensure a smooth transition to community-based therapy and increase access to available resources. 

Services:
-In-person or virtual visits with a licensed behavioral health provider
-Diagnostic evaluations to aid in clarification and treatment planning with your medical provider
-Cognitive screening in relation to concerns around dementia or head injuries
-Same day access for urgent needs
-Support with getting connected to specialty mental health and substance use treatment in the community
