Annual Wellness Visits:
Many insurance plans provide coverage for an "Annual Wellness Visit" or "Annual Physical," and often there is no co-pay or deductible associated with these visits. However, please be aware that these visits are not intended to diagnose or treat significant health conditions. When coded and billed properly to insurance, patients may incur a cost for these visits. Please check with your insurance company for details specific to your plan.

If you do not show up for your scheduled appointment and you have not notified us at least 24 hours in advance, may be subject to cancellation fees.
